What is cellulite?

Cellulite is described as a harmless condition that affects the fat collections and connective tissue underneath the skin. When the collected fats apply pressure into the connective tissue, the outermost layer of the skin appears puckered or bumpy. Cellulite is more common among women as compared to men. This condition is not contagious.

What are the symptoms and causes of cellulite?

The main symptom that gives away cellulite is the unevenness of the skin’s surface. At first glance, cellulite can be identified by dimples or bumps in the surface area of the skin when it is pinched. The texture is similar to a peel of an orange. Cellulite can appear in the following parts of the body:

  • Abdomen
  • Breasts
  • Buttocks
  • Hips
  • Thighs
  • Upper arms

There is not much known about the causes of cellulite, but some factors such as aging, gender, genes, hormones, lifestyle, muscle tone, pregnancy and weight contribute to its development. Cellulite also affects individuals who are physically fit, contradicting the popular belief that this condition is only prevalent among those who have excessive fats in the body.

To know the severity of your cellulite, the condition is categorized into four grades:

  • Grade 0 – Cellulite is not present in any part of the body
  • Grade 1 – Appearance of slight dimpling on the surface of the skin when you are standing, but the skin is smooth when standing on your feet.
  • Grade 2 – Dimpling in the skin is more visible when sitting down or standing up.
  • Grade 3 – Dimpling in the skin is profound with obvious depressions and lumps in the surface of the skin.

What are the available cellulite treatments in Singapore?

There are several treatments for cellulite that are available in Singapore. 

If you prefer a treatment that can be done at home, you can opt for topical creams. Topical creams can work in two ways:

  1. Skin hydration
  2. Skin dehydration

Topical creams that promote skin hydration have aminophylline and retinol primary ingredients. These two elements improve hydration in the skin, which allows it to become more pudgy. On the other hand, topical creams that contain aminophylline and caffeine do the opposite. These components promote dehydration by way of removing the water in the fat cells, which contracts the blood vessels, decreasing the lumps in the skin. While topical creams can be effective in reducing cellulite, its results are not permanent.

For long-lasting results, you may opt to look into various invasive and non-invasive cellulite treatments.

Invasive options include:

  • Cellulite subcision 
  • Polylactic-L Acid (PLLA) injectable

Cellulite subcision is done through a surgical method where bands of fibrous tissues that pulls the fat cells together creating the cellulite are cut to release the skin. Cellulite subcision is performed by a doctor specifically trained for undertaking this surgical procedure. The treatment is done following these steps:

  • The area in the skin where the bumps and dimples are obvious is marked while the patient is standing.
  • Tumescent anaesthesia is then injected into the deep and superficial fat layers of the treatment area. Tumescent anaesthesis plays a vital role in this procedure as it protects the flesh from injury while keeping the patient’s discomfort at minimum. The solution likewise helps in lessening the development of bruises in the skin.
  • A non-coring needle is then inserted into the skin, reaching the subcutaneous tissue at a depth of 1.5 to 2 cm and cutting the fibrous bands. 

A recovery time of at least two to three weeks is needed following the procedure. Patients will be required to use compression garments for the complete healing of the treatment area. Bruises may appear in the skin, but these should clear after a week or two.

Polylactic-L Acid (PLLA) injectable is a minimally invasive procedure that uses fillers. The most common brands known available in Singapore are Dermaviel, Lenisa, and SCULPTRA. Among these, only SCULPTRA has obtained a license for clinical use. To improve the skin’s appearance, PLLA is injected into the skin and fills in the hollow and depressed areas. In addition, PLLA also stimulates collagen production, which also improves the skin’s elasticity. Anesthesia is used prior to injecting PLLA, so patients can expect minimal pain during the procedure. Improvements can be seen instantly for moderate cellulite cases, though more serious conditions may need more than one session to see notable and lasting results. 

Non-invasive options for cellulite removal are:

  • Radiofrequency Treatment
  • Vacuum Suction Treatment

Radiofrequence (RF) treatment uses a powerful equipment that delivers RF heat deep into the skin’s tissues. The heat energy then stimulates the formation of new collagen, which tightens the skin that surrounds the cellulite, making its appearance less visible.

Vacuum suction treatment uses a device that stretches the fibrous tissues that cause the cellulite. This stretching makes the blood circulation and lymphatic drainage to function better, releasing fluids from the fat compartment. As a result, the dimpling in the skin is reduced.
